New Patient Forms

Completing your paperwork in advance makes your first visit faster. The intake packet covers your medical history, current medications, insurance information, and HIPAA acknowledgment.

Patient Portal

Our patient portal is powered by athenahealth. Sign in to message the office securely, view your visit summaries and lab results, request prescription refills, review upcoming appointments, and update your information — all without having to call.

Financial Assistance

Sulphur Surgical Clinic partners with CareCredit to give patients access to flexible, no-interest and extended financing for out-of-pocket medical expenses. CareCredit can be used for deductibles, copays, and procedures not fully covered by insurance.

For patients who are uninsured or facing financial hardship outside CareCredit's scope, we also offer self-pay pricing and case-by-case payment arrangements.

What to Bring to Your First Visit

• Photo ID and current insurance card(s)
• Referral form if your insurance requires one
• Records or imaging from prior providers, if related to your visit
• A list of current medications, including doses
• The completed new patient intake form (if you received it in advance)
• A family member or friend, if you'd like a second set of ears
